Before NASA could send astronauts around the Moon for the first time in half a century, it had to answer one question honestly: why did the Orion heat shield come back from Artemis I missing pieces? In May 2024 NASA’s Office of Inspector General published its answer - IG-24-011, NASA’s Readiness for the Artemis II Crewed Mission, the audit that made the char-loss anomaly public in detail and forced the program’s most consequential safety debate. This volume reproduces it in full facsimile, photographs included. The report documents what the uncrewed Artemis I flight revealed: more than a hundred locations where the ablative Avcoat material cracked and liberated from the shield in ways the models did not predict - fortunate on an empty capsule, intolerable with a crew aboard. The OIG traces the investigation into root cause, the trade space NASA faced between reentry-trajectory changes and hardware modification, and the schedule arithmetic binding the heat shield decision to launch readiness. Beyond the shield, the audit inventories the remaining risks of the first crewed flight - life support systems flying with astronauts for the first time, the launch abort system’s qualification envelope, SLS production tempo - and the cost trajectory of a program whose per-flight expense the OIG had already flagged as unsustainable.History gave the report its vindication: the heat shield debate it surfaced played out exactly along the lines it mapped. As Artemis II recedes into history, this document remains the contemporaneous record of NASA deciding, under audit scrutiny, how much risk a return to the Moon was worth. The official source document is reproduced in full facsimile, with separate editorial apparatus of the public document, produced under 17 U.S.C. §105. For aerospace engineers, human-spaceflight historians, risk managers, and every reader who watched the launch wondering what the debates had been, IG-24-011 is the inside account, on the record. The report’s afterlife made it a case study in oversight working as designed: publication forced the heat-shield deliberations into public view, congressional hearings adopted its framing, and NASA’s eventual flight rationale answered the questions in substantially the form the OIG posed them. For risk-management educators, the document offers something rare -- a complete, contemporaneous record of a life-critical engineering decision, including the evidence, the options, the institutional pressures, and the dissents, published before the outcome was known. However Artemis II is remembered, this audit preserves what was actually known and argued at the decision point, which is precisely the record hindsight always erases.About this series: Nimble Ultra’s American Spacepower Documents collection puts the primary sources of the space age’s second great competition into durable, uniform print -- doctrine, strategy, intelligence, oversight, and exploration architecture, each volume an exact and complete facsimile of the official public release, with full bibliographic identification for citation. Government URLs break, documents are revised without notice, and libraries deserve stable copies; these editions fix the record as issued. Collect the doctrine shelf alongside the analytical flagship of the series, the RKHS-extended edition of Space Force Vector 2025.This 54-page Nimble Ultra paperback identifies NASA Office of Inspector General as the originating body and preserves NASA’s Readiness for the Artemis II Crewed Mission to Lunar Orbit (IG-24-011) as a durable print reference. The edition is intended for policy research, technical study, teaching, professional consultation, and library collections.

Attitude control of the rocket is a big challenge in real-time. To make the rocket stable against the influences, this book has examined the control technology such as proportional integral derivative control using an anti-windup mechanism and linear quadratic regulator theory based on the degree of freedom mathematical model. The transient behaviour of both controllers is not ...
