La cuestión del Doble está íntimamente relacionada con la de la locura. Sin duda fue este libro el que impulsó a Freud, unos años más tarde, a escribir su Unheimliche en el que cita a Rank. La cuestión del doble atraviesa todo el trabajo de Rank. Este ensayo de Rank me permitió ir más allá en el análisis del complejo de Caín: ¿Cuál sería la razón del odio hacia el hermano o la hermana? Proponemos esta interpretación: el hermano o la hermana son encarnaciones del doble, encarnación aproximada que puede llegar a ser extrema en el caso de los gemelos, y que deben ser asesinados porque manifiestan el doble monstruoso, portador de la amenaza de la locura y por tanto de la angustia. Esta angustia, este miedo al doble, está en la raíz del complejo de Caín. Rank señala que la civilización puede haber comenzado al superar la ansiedad del doble, y aceptar así la relación con el similar, con el hermano, para construir la ciudad.
